Kim Kardashian has shared her journey with #psoriasis on her sister’s lifestyle website Poosh (link in bio). Someone with such a high profile speaking so candidly about living with a long term skin condition will hopefully be very comforting for others who are going through the same. I think there is a strong #skinpositivity message in her story and I’m very happy if this gets more people talking about psoriasis, or indeed asking for help if they need it. As a Dermatologist though, I do have concerns that people (who may be vulnerable) will try to follow the same advice, for example salon tanning devices, teas and sea moss smoothies, particularly as the website’s Instagram caption says “read what really works”. I would urge anyone who is curious to please speak to a doctor or a verified support organisation to find out which treatments are safe and have scientific data confirming their effectiveness before you experiment with options that are at best unlikely to help and at worst potentially dangerous. In writing this post, I am also extremely conscious of not sounding too judgmental as I have not been in her shoes and her journey is her journey. Wow - this unexpected and wonderful surprise happened this evening! I am honored and humbled to receive the Physician Humanitarian Award for my hospital. One physician is selected each year at our annual Physician Celebration Dinner to receive this honor. I always enjoy this event to network with my colleagues and learn more about them. Considering the accomplished past recipients of this award, a part of me does not feel worthy, but I have to remind myself that I do my small part to make the world a better place and am grateful to be recognized and appreciated. 🙏 . Today, I failed a patient. It has been weighing heavily on my mind and heart. Going into the celebration dinner it is all I could think about. Hopefully, I’ll still have the opportunity to improve and do right by the patient. . Another patient reached out and said she wants to give me some honest feedback about my practice. Never an easy thing to hear, but I value her insight and feedback. It gives me an opportunity to learn and grow so I respect her and am grateful that she is sharing her thoughts. . It’s been a tough day. . Then this amazing thing happens and reminds me that it is ok to trip and fall, but getting up and moving forward is where the growth and progress occurs. . A year ago I developed a physician wellness and burnout prevention program at my hospital which is gaining visibility and momentum. 50% of physicians are experiencing burnout and 88% are stressed out. It is important that physicians come together to connect, communicate, collaborate, build a community, and create a culture of wellness. We educate, inspire and care for our patients daily so it’s time we did the same for each other. By supporting one another and lifting each other up, we’ll find a path to fulfillment and truly thrive. . Dear medical students, The first time I got into an OR I fainted. . I was a med 3 student so excited about witnessing my first operation (cant remember what it was though!🤷🏻‍♀️). I put on scrubs, head cover and mask 😷! I was not gonna scrub on this one, but it was still a big deal for me! . I dont think it’s the blood that bothered me (I have never been afraid of blood ever) but maybe the cold atmosphere of the operating room. I’m not really sure what it was, but Quickly I got a bit dizzy, and pretended I felt fine when the nurse asked me. . Its only few minutes later that I almost fell on the floor, caught by the anesthesiologist 🤕. I was taken out of the room, someone got me water, and all I can remember is that I kept on saying the reason I fainted was because I did not have breakfast (I’m pretty sure nobody bought this excuse 😂) . At that time I felt so stupid and embarrassed, I thought I would never set foot in an Operating room in my life! I didn’t want to be a surgeon yet, but still felt like this was the end of the world 😢 . 12 years later I am an OBGYN, spending a lot of time in the OR and I can assure you that I never fainted again! . This is one of the failures I had to face early on in my training, with so much more to come! Just remember that what might seem like a failure today, will be long gone years later. So don’t give up at the first bump, cause it could actually be what you need to improve and succeed! I’ve been feeling flustered and stressed and a little out of control. I know my routine (or lack thereof) is partially to blame. Wikipedia is not a reliable source. It is useful, but full of bias and opinions that sometimes can be changed and other times locked based on the beliefs of the people who run Wikipedia. Check where you obtain your information. Be a critical reader and a critical thinker. Do not believe everything at face value. This is what led me to the path of discovering Naturopathic Medicine. I was told there was nothing I could do to cure my migraines and that my only option, or the only tool all these highly intelligent professionals had for me were medicating me with daily injections for the rest of my life. 🚨Emergency 🚨. . . This word means different things to different people. Last night I had the opportunity to shadow a doctor at a specialist emergency clinic 🏥 in our area. . . Never heard of a specialist emergency clinic? Me neither! The short story is, it’s a clinic that was set up by a few emergency specialists to capture those people that are too sick for a walk in clinic 🤕 but would probably spend a really long time waiting in ED 😷 and not really need all the services ED can provide. It really is an interesting and innovative way to cut down the strain on ED and wait time for patients ⏱. . . Anyway during my visit I got to see everything from a pretty bad flu 😷 , extreme tonsillitis 😖, a couple broken arms (kids are tough 💪 I tell ya!), a potential spider bite 🕷, the list goes on! . . Seeing all of these patients, and their loved ones, really got me thinking about the word emergency 🤔. . . The doctor I shadowed last night had excellent bedside manner so he never belittled a patient or their worry 🙁. But he and I did have some conversations about how much of what we saw last night - while exactly what the clinic is there for - isn’t quite an “emergency” 🚨. . . This really made me think 💭 about how lucky we are as doctors 🥼(or will be as doctors). We will have the knowledge to know when something is a true life threatening emergency 🏥(and intervene). But we will also be able to sit with patients and their family in what may be an emergency to them and reassure them 👍. Just because it’s something “minor” to us as doctors doesn’t make it any less scary for them, and we have the pleasure to help calm those fears ☺️. . . A little throwback to nursing school round 1️⃣ before I had a clue. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Some days — I still do not have a clue. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ But with each experience comes having more than a “clue” how to navigate that specific situation next time. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ That is what I seek during each 12-hour shift - alllll dat experience and alllll of those learning opportunities. Without em’ I simply would not GROW as a baby nurse. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ If I can give one piece of advice to any nursing student or new nurse it would be to INSERT YOURSELF into any and every situation at work/clinical. No matter how shy you are. The worst thing that could happen is that you’re asked to leave due to crowding. I 🖤 coffee!! ☕️(Not pictured, three carafes of coffee yet to be inhaled). Today, one of my patients with type 2 diabetes asked me if it is ok to drink coffee...or La Croix...or goat meat...”and what about carrots?” I was running behind on patients, but it was clear that, although I had discussed a DM diet with this patient before, we never really dug deep. So, I stayed the extra 10 minutes (with an interpreter), and we dove DEEP! . . . Sometimes, health literacy takes time. This patient came to the U.S. as a refugee nearly 5 years ago. She has come such a long way, learning how to navigate the bus to her appointments, getting to the pharmacy, using a glucometer, and tracking her blood glucose levels. As we know, though, no matter who you are, there is ALWAYS room for improvement. . . . Today I was talking to some classmates about how PA students are taught differently depending on which school they attend. At Stanford, for instance, most lectures are taught by experts in the field (MDs, PharmDs, PAs, PhD candidates, etc.), while only a few are taught by the core faculty. There are pros and cons to this. Pros: - You will learn the cutting-edge research that is taking place in the field as well as each specific disease process in great detail - You will also learn more than what is expected of you as a typical PA student, which will make you more knowledgeable and a better clinician down the road Cons: - Sometimes the lectures are too detailed for what you’re expected to know for the PANCE - When being taught by researchers who are clinicians as well, the lectures can have a research-focus as opposed to a clinical-focus. - There is less continuity in the teaching (different expects are teaching each topic) On the other hand, at other PA programs each organ block is taught by the PA core faculty. Pros: - The core faculty are aware of what you need to know to pass the PANCE and will ensure each lecture is relevant and has a clinical focus. - There is continuity in the teaching (one person is teaching each organ block) Cons: - You are not expanding your breadth of medical knowledge as much as if you were taught by experts in the field - You will be less aware of the cutting-edge research taking place in each field I personally don’t believe one way of teaching is better than the other, but I do think it’s important to be aware of these differences. During your interviews, you may want to ask how many lectures are taught by guests versus core faculty. I hope this information was helpful. Didn't have time to collect myself before starting my day which is frustrating. Sometimes you just have to mentally prepare yourself for work, ya know? A lot of residency has been like that especially on the days when I'm on-call. 😅 1st day of a new rotation in Rehab Consults, and I'm pooped. Wasn't even a particularly busy day but everyone wanted everything at once this morning and then everything got pushed back. Late with rounding and didn't get home until 7:40pm! Later than some inpatient rehab days 😬 And of course, I skipped my workout this morning, so I had to get that done. It's just one of those Mondays... But I know this won't be forever, so I just have to keep chugging along!

Medical school is weird. It has me walking around my apartment thinking about all the muscles 💪🏼 (including blood supply and innervation) I use even when I’m just reaching for the cereal box on top of my fridge. It’s a long list of them, btw. At the end of last week, we dove into the brachial plexus and the different compartments of the arm. BOOK REVIEW: A colleague asked me to review this book b/c it was published this year and has no reviews of it yet online. This is the Canadian version of the original US book by Dr. Rackner, surgeon and Dr. Varenbut, an ?ex- addictions doctor (I'll let you google them yourselves). This is a good BEGINNER book for anybody working in healthcare (not just doctors but pharmacists, nurses, PAs) . ✅ PROS: 1️⃣ Addresses the taboos of why doctors don't talk about money and dives into this uncomfortable conversation in the first chapter. 2️⃣ Covers a wide breadth without a lot of depth: i.e. the 'money talk' before marriage, medical side hustles or alternative practice ideas, personal & practice finance...etc. This provides a broad overview with room for deeper research later in practice. 3️⃣ Dumb Doctor Deals: discusses common ways healthcare providers lose money, e.g. discomfort with talking about fees, fraud. . 📉 CONS: 1️⃣ Lacks depth: since it skims over many financial topics, you won't find detailed steps on how to invest or start up a practice here. 2️⃣ Needs editing: while copy/pasting, some text were copied into the wrong section (swipe 👈) . 👩🏻‍⚕️ I think the most important lesson about money & medicine can be learned outside of the pages of this book (if you googled the authors). The key lesson highlights a difference between business and medicine - in business we can achieve economies of scale, do things cheaper, faster and even get away with deceptive marketing campaigns. However, not all lessons in business are replicable in healthcare b/c there are immense risks in putting a profit to people’s lives. . 💁🏻 Have you read this book? What is a good medical/finance book you recommend?

the past couple weeks have been strange. i act as the nurse at work all day and then go home and become the family member to a patient. my mom has been in and out of the hospital (and is unfortunately back in). while sitting with my mom over the weekend, i listened to a conversation between the nurse and a physician and i have not been able to get that conversation out of my mind. • while discussing my mom’s condition, the nurse never referred to my mom by her name, or even “she” or “her”. my mom was only called “the patient”. she sat right there and was called “the patient” directly to her face. i know it’s clinical and easy to say, but listening to my mom only be called “the patient” felt almost insulting. my mom, and everyone receiving care, is more than just a patient. and as nurses, we should be treating the person and not just a condition. every person you care for is someone’s parent, child, sibling or best friend. and they deserve to be treated as such, and called by their name. we need to do better. and we need to make sure our patients know that they are more than just “the patient” to us.

It is for this reason it commonly used on a day to day basis by people saying that they are depressed or labelling others as depressed. People are allowed to be sad or down doesn't mean they are depressed. To actually suffer from depression entails alot more than just feeling down. It affects every aspect of ones life from the ability to work to having difficulties performing simple tasks like eating and communicating with others. If you know anyone suffering from depression please dont make assumptions and dont label just because it's a mental illness. Educate yourself before speaking. Depression doesn't choose who it wants to affect. It can happen to anyone of any race gender or ethnicity. It can happen to those with millions of dollars that have everything and it can happen to those that have nothing. Yes being in exposed to certain stimuli can predispose you to getting depression but most times people get depression just the way others can get cancer!!! This ended up being super long. But mental health is commonly stigmatized and people are inappropriately labelled which affects their health seeking behaviours. Nobody wants to be labelled as the looney person or the attention seeker. They also just want to live a normal life. It's a disease just like any other. Just because it physically can't be seen doesn't mean it's made up. Happy Monday!! These past few weeks have been crazy for me- from surviving my first month of inpatient medicine wards, transitioning into night float, starting wedding planning, research deadlines, residency educational modules, conferences, etc. Granted all of these are things I love and I’m grateful to be moving forward on my journey. Nearly 10 years ago I committed to a career in medicine. On the left is my high school graduation and on the right my medical school graduation! I knew from a young age that I wanted to become a physician. Training in a combined BS & MD program allowed me to foster my passion for healthcare early on and connect with the unique patient populations in my community on a deeper level. I remember feeling nervous about moving to a big city and committing to such a long path of training that would take up a great portion of my 20s. Now I find myself looking back at how quickly the time has passed, all of the knowledge/skills I have gained, the phenomenal people I have met along the way, and all of the amazing memories created in South Florida. I can confidently say I wouldn’t have done it any other way.
